I totally agree with Ralph, a good discussion ( for later)  =3Bis needed on " what to do with the data and how to share?"

The difference with an incident is that there is no loss or ($=0)

I can see lots of potential uses, but it's unlikely that you'll gather statistically useful information with a random data collection system. People will have to see it being used usefully in order to be motivated to provide the information you're after. To me, this suggests that sharing the stories in a community honored way (i.e. prizes) is one possible use.
