I believe we will make better progress when we stop talking about "safety" (nebulous) & start talking about "safe work performance." This encompasses job planning, hazard analysis, documented & verified procedures & adequate training of those performing the work. Then it becomes a process with a predictable outcome & not just saddling a person with "being safe." People make mistakes, but a robust process can prevent many from occurring/repeating.
