Anyone with the potential to receive >100mRem/year is given a TLD after undergoing special training. As in the hospital setting, the TLD should be worn at work in the facility at all times: you cannot predict when an over exposure might occur! This also lets you immediately pick out those who have taken off their badge when they get too close to their max ( Interventional Radiologists and Cardiologists are notorious for this..... In special circumstances and by exception, I've also issued a a TLD to anyone who has special circumstances: for example a pregnant woman where she and perhaps the supervisor want the psychological comfort of objective negative data.
